| DOCUMENTS NEEDED: | Certified Copy of Birth Certificate Parent/Guardian Photo ID Physical Form (physical must be within the last 12 months prior to the first day of school) Proof of Residence (utility bill, lease, mortgage statement) A notarized residency affidavit is required if living in another household |
